(“GAHR IV”), Griffin-American Healthcare REIT IV Holdings, LP, Continental Merger Sub, LLC (“Merger Sub”), Griffin-American Healthcare REIT III, Inc. (the “Company”) and Griffin-American Healthcare REIT III Holdings, LP, the Company merged with and into Merger Sub on October 1, 2021, with Merger Sub continuing as the surviving entity and a subsidiary of GAHR IV, at which time the separate corporate existence of the Company ended.
Pursuant to the requirements of the Securities Exchange Act of 1934, Continental Merger Sub, LLC, as successor by merger to Griffin-American Healthcare REIT III, Inc., has caused this certification/notice to be signed on its behalf by the undersigned duly authorized person.
